Birth Certificates

The Toledo-Lucas County Health Department can provide certified copies of Birth Certificates for any person who was born in the State of Ohio. In the past, you would have to obtain the certificate from the county of your birth. Statewide Issuance gives you the ability to obtain your Birth Certificate without the inconvenience of traveling to your birth county or applying by mail. You can now easily obtain the certificate in Lucas County only at our downtown Toledo location.

Important: Non-certified copies are no longer issued. The complimentary birth record that is given at the hospital is not an official record and cannot be used for legal identification.

Certificate Fees

The fee for certified copies of birth or death certificates is $25 per copy. Methods of Payment include cash, check, credit card, or money order. Credit Card purchases require an additional processing fee. Due to changes in State law, we can no longer provide uncertified copies of birth or death certificates.

How to Order

You may order copies of certificates by phone, mail, in person, or online with a credit card.

By Phone: You can now order your Birth Certificates by phone using a major credit card (Discover, MasterCard, or Visa). Call 419-213-4100 to place your order. In most cases, certificates will be mailed the same or the next business day. Please allow 7-10 days to receive via USPS. *An additional fee will be applied if a credit card is used.*

In Person: Visit the Vital Statistics Office at the Toledo-Lucas County Health Department. In most cases, your certificate will be processed and issued in about 15 minutes. This service is offered at our Downtown Location only.

By Mail: Mail the completed Birth/Death Record Request Form along with payment for the certificate and a self-addressed stamped envelope to Vital Statistics at the Toledo-Lucas County Health Department at 635 N. Erie Street, Toledo, OH 43604. Upon receipt of your request, your certificate will be mailed back to you within 2 business days. Please allow 7-10 business days for the return mail. When requesting by mail please include the following information: Name on record, Date of birth, County of birth, Mother’s maiden name, and Father’s name.

Online: For your convenience, you can process online requests through VitalChek Network, Inc. This is an independent company the Toledo-Lucas County Health Department has arranged to provide you with this service. This option allows either overnight shipping or two-day shipping via UPS.

VitalChek may be reached through their website. An additional fee of $7.00 (plus shipping) is charged by VitalChek for this service. All major credit cards are accepted, including American Express, Discover, MasterCard, or Visa.
