Overview

The Sixty Plus Program was developed in 1978 by staff of the nursing division of the Toledo-Lucas County Health Department (TLCHD) in response to documented community need. The program is jointly funded by the Area Office on Aging and the TLCHD.

Program Goal

The goal of the 60+ Program is to provide health promotion and disease prevention to senior residents of Lucas County and to help them maintain their present level of independence.

Health Assessments

Health Assessments are provided at a variety of locations in Lucas County. These include health and medication history, weight, pulse rate, blood pressure, and blood sugar (if indicated). Regarding results of the assessment process, proper use of prescription medication, good nutrition, exercise, and health risk factors are reviewed with the individual client. Referrals are made when indicated to a primary care physician or other appropriate source of care.
